OT: Aetna, Tricare, UHC, Sunshine, Simply, Cigna
We are out of network with BCBS for OT/Speech Therapy.
Scholarships
We are a direct provider of the Family Empowerment Scholarship. This is the formerly Gardiner Scholarship Program.
Lama Pediatric Therapy collects estimated amounts of your deductible, copayment, or coinsurance. Invoices are sent to our patients via email on a bi-weekly basis.
Invoices will be sent from Quickbooks to the email address on file. We will also ask that all patient have an updated credit card authorization form on file to be charged bi-weekly of the estimated total amount in order to continue to receive services.
Payments are due within 48-hours from the invoice date. We accept payment from Visa, American Express, Discover, Mastercard, Health Savings Accounts (HSA), and Flexible Savings Accounts.
We do not accept payments in the form of cash or checks.

Insurance Coverage Change
If your insurance changes, please notify us before any future appointments as prior authorizations may be required. If your insurance company does not pay your claim, the balance will automatically be billed to you.
