Scholarships
We are a direct provider of the Family Empowerment Scholarship. This is the formerly Gardiner Scholarship Program.
Lama Pediatric Therapy collects estimated amounts of your deductible, copayment, or coinsurance. Payment is due at the time of service.
We ask that all patients have an updated credit card authorization form on file to be charged weekly of the estimated total amount in order to continue to receive services.
We accept payment from Visa, American Express, Discover, Mastercard, Health Savings Accounts (HSA), and Flexible Savings Accounts.
We do not accept payments in the form of cash or checks.

Insurance Coverage Change
If your insurance changes, please notify us before any future appointments as prior authorizations may be required. If your insurance company does not pay your claim, the balance will automatically be billed to you.
